**This is for those students that have not yet had their Year 9 Immunisations/parents that have not in the past completed a consent form for this year’s Year 9 Immunisations.**

Spring Term 2019
Dear Parent / Guardian,
Vaccination against Diphtheria, Tetanus, Polio and Meningitis ACWY in Year 9
This letter is to advise you that the Immunisation Team will be visiting all secondary schools in Devon to vaccinate children against Diphtheria, Tetanus, Polio and Meningitis ACWY as part of the National Childhood Immunisation Programme.
Diphtheria, Tetanus and Polio are given as one combined vaccination and Meningitis ACWY is given as a separate vaccination at the same time. These vaccinations complete the routine immunisation programme (which is usually started as a baby) and provides longer-term protection against these serious diseases.
We are now inviting parents / guardians of children born between 1st September 2004 and 31st August 2005 to register their consent for the above vaccinations.
In response to parent feedback we have developed a secure online link that parents can use to register their child for the vaccination. This process is quick and easy to use, and comprehensive information relating to the vaccinations will be accessible via this portal. The link provided below is unique to each school and should not be used for any child registered at a different school.

We will be visiting your child’s school during the coming Spring or Summer Term and will require completed online consent by 10th February (now extended until Sunday, 26 May 2019). If your child has a medical condition we may need to contact you or your child’s doctor for further information before we can proceed with the vaccination.
